Work Permit Application
Work Permits should be obtained at the school at which the student is attending. Permits can be obtained at all CB middle schools and high schools.
Work Permits are available for pick up during the school day at the Reception desk in the lobby. In summer months, please go to the main office. Summer office hours are M-TH 8:00am-2:30pm).
Please complete the work permit application. The application must include a parent/guardian signature.
Bring a form of ID/proof of age. Acceptable forms of ID include driver's license, birth certificate, passport, baptismal certificate, or any other official documentary evidence indicating age/birth date of minor. A school issued ID is not an acceptable form of identification.
The minor requesting the application must be present to sign the permit.
